Clara Novak Joins JFS
Clara Novak, LSW has joined JFS as a care coordinator on the UCare team. In this role, Clara will provide comprehensive case management services for seniors and individuals living with disabilities who are enrolled in Minnesota Senior Health Options (MSHO) and Minnesota Senior Care Plus (MSC+) state health managed care plans that cover health care and Elderly Waiver services for people who are 65 years old and over and for people who are on Medical Assistance.
Clara is a graduate of University of Wisconsin and has worked in early education in the Twin Cities.
