Tamara Chapman Joins JFS
Tamara Chapman, LSW has accepted the position of Medica Care Coordinator at JFS. She will be responsible for monitoring a caseload of clients over the age of 65 and conducting annual assessments to determine eligibility for additional home and community based services. She is also responsible for developing care plans for her clients to address goals and needs and assist in coordinating and setting up services and supports through the Elderly Waiver.
Tamara comes to JFS from Senior Linkage Line where she was responsible for visiting clients in the metro area to connect them with community resources.
